SHIPPING POLICY

At Formula Integrative Health and Wellness, we offer high-quality wellness products and functional lab testing kits, fulfilled and shipped directly through our trusted partner, EquiLife.

Domestic Shipping (U.S. Only)

  • Orders are typically processed within 24 hours, Monday through Friday.

  • Products are shipped from EquiLife’s Chicago-based fulfillment center via FedEx.

  • A shipping confirmation email with tracking details will be sent once your order is on its way.

Domestic Shipping Rates:

  • FREE standard shipping for orders over $250

  • A flat rate of $14.95 applies to orders under $250

International Shipping & U.S. Territories

We currently offer automated checkout and shipping within the continental United States only. This excludes Alaska, Hawaii, Puerto Rico, and all international locations.

If you are located outside the continental U.S., please contact us prior to placing your order for a custom shipping quote and manual ordering assistance.

Note: Shipping charges, international taxes, tariffs, and VAT (where applicable) will be calculated and provided before order confirmation. We’ll help ensure accurate delivery and compliance with import regulations.

RETURNS POLICY

We want you to feel fully supported in your wellness journey. If your product arrives damaged, defective, or incorrect, please follow the return process below:

Return Process:

  1. Contact EquiLife Customer Support at support@equi.life or 888-348-8890 (Mon–Fri, 9:30 AM – 4:00 PM ET) within 30 days of your purchase to request a return or replacement.

  2. EquiLife will review the request and, if approved, facilitate the return process.

  3. Once EquiLife issues a refund to Formula Integrative Health and Wellness, we will promptly refund you via your original payment method.

Important Notes:

  • Returns must be approved and processed through EquiLife.

  • Refunds will only be issued after Formula Integrative Health and Wellness receives reimbursement from EquiLife.

  • Opened, used, or activated lab kits are non-refundable.

  • Refunds will not be granted for buyer’s remorse or dissatisfaction unrelated to product condition or quality.

We recommend saving your order confirmation email and tracking number for reference during any return requests.
